Proof of ANATOMY education, OR highest education if you do not have any
anatomy
1. This can be a transcript, diploma or certificate)
2. This must include YOUR NAME and NAME OF ISSUING INSTITUTION

Only include CLEAR, SIZE APPROPRIATE and PRINTABLE Attachments. Review your PDF before uploading to confirm it meets requirements.
